The following information is needed in order to enroll a child/student in Scribbles:
- Registration form (downloadable)
- Child’s ‘physical’ form from doctor – (including immunization records)
- Certified birth certificate
- Doctor’s note indicating any food allergies or medications to be administered by Scribbles staff
Prior to starting at Scribbles, families will attend a parent orientation to review the center’s policies and procedures with an administrator.
We can accept forms by email or fax (309.665.0011), but please call and let us know that they are on the way so that we can be sure to expedite this process as much as possible for your convenience.
