The following information is needed in order to enroll a child/student in Scribbles:
- Registration form (downloadable)
- Child's 'physical' form from doctor - (including immunization records)
- Note from doctor for any food allergies or medications to be administered by Scribbles staff
Prior to starting at Scribbles, families will receive a copy of an admission agreement to review and sign.
We can accept forms by or fax (309.665.0011), but please call and let us know that they are on the way so that we can be sure to expedite this process as much as possible for your convenience.
